The Plasma Concentration Levels of Adrenomedullin in Egyptian Patients Complaining of Mitral Stenosis, With and Without Pulmonary Hypertension

چکیده
Mitral stenosis is associated with increased pulmonary artery pressure and resistance. Adrenomedullin, discovered in human pheochromocytoma tissue in 1993, has shown to dilate the pulmonary vessels and so may participate in the control of pulmonary circulation. In this study, we assessed the plasma concentration of adrenomedullin in patients complaining of rheumatic mitral stenosis, with and without pulmonary hypertension. The study revealed a significant increase in the plasma concentration of adrenomedullin, in patients with rheumatic mitral stenosis without pulmonary hypertension and highly significant increase in the plasma adrenomedullin concentration in patients with rheumatic mitral stenosis and pulmonary hypertension. The study revealed also a significant positive correlation between the plasma level of adrenomedullin and mean heart rate, mitral valve area and peak pulmonary artery systolic pressure in all cases of mitral stenosis, with and without pulmonary hypertension. Inspite of the increased production of adrenomedullin, supplement of adrenomedullin would be of beneficial effect in improving deteriorated conditions.

OBJECTIVE To examine the pathophysiological significance of adrenomedullin in the pulmonary circulation by investigating the relation between plasma concentrations of adrenomedullin and central haemodynamics in patients with mitral stenosis. METHODS Plasma concentrations of adrenomedullin in blood samples obtained from the femoral vein, pulmonary artery, left atrium, and aorta were measured b...
متن کاملThe active molecular form of plasma adrenomedullin is extracted in the pulmonary circulation in patients with mitral stenosis: possible role of adrenomedullin in pulmonary hypertension.
Adrenomedullin (AM), a novel hypotensive peptide, preferentially dilates pulmonary vessels rather than systemic vessels. This suggests the possibility that AM is a circulating hormone which participates in regulation of the pulmonary circulation. A recent study revealed that two molecular forms of AM, i.e. a mature, active form of AM (AM-m) and an intermediate, inactive, glycine-extended form o...

Six patients with mitral stenosis complicated by renal artery embolism studied prospectively are presented. Adrenomedullin, a potent hypotensive peptide, reduces blood pressure and pulmonary vascular resistance, and increases pulmonary blood flow. The mRNA for adrenomedullin and its receptor is highly expressed in the lung, suggesting a regulatory role for adrenomedullin in the pulmonary circulation. To investigate the clinical significance of adrenomedullin in patients with pulmonary hypertension, w...
